The Upper Connecticut Valley Hospital keeps a confidential medical record about you. The record includes medical information from doctors and other hospital staff at Upper Connecticut Valley Hospital.

If you need information released, the Health Information Management Department will send information for you. We want this process to be easy for you while still protecting your privacy. Most often, that means we need your request in writing. You may send your request to the Health Information Management Department, or drop it off in person. We are available for this 8:00 a.m. to 4:30 p.m. Monday through Friday.

We charge a processing and photocopy fee in certain instances. If you have questions about our release of information policies and fees, please call the Health Information Management Department at (603) 388-4300.
